Mission for the Foundation for Care Management – a center for learning and change

We endeavor to directly and jointly provide programs that facilitate changes in patient outcomes.  We measure this through program evaluations and conduct outcome studies to gauge overall program success articulated through changes in knowledge, skills, performance and competence for individual learners and team based collaborations.

Evidenced-based outcomes expected of participants who complete FCM-provided programs include:

  • Enhanced team-based learning (IPCE) as a result of current continuing education in broad-based knowledge that is directly tied to improvements in skills, strategy and/or performance
  • Effective and accurate treatment of diagnosed conditions, e.g. accurate and appropriate pharmacotherapy leading to improved quality of life
  • Improved patient and/or community health or well-being tied to the top ten chronic disease states that affect Americans today
  • Improvements in performance and skills strategy based on current evidence based literature in a wide range of topics and specialties to enhance the work of the team and improve patient outcomes

To ensure expected outcomes FCM engages in continuous quality improvement by

  • reviewing program results and making needed adjustments to ensure its mission is being met to its fullest extent
  • conducting weekly internal meeting to review internal systems and workflows to increase efficiently as it relates to information gathering and analysis of information
  • ongoing education through the ACCME Academy and other educational offerings
