About FCM

Mission Statement

The Foundation for Care Management’s mission as an accredit provider is to uphold the standards as designated by the:

  • Accreditation Council for Continuing Medical Education (ACCME)
  • Accreditation Council for Pharmacy Education (ACPE)
  • American Nurses Credentialing Center (ANCC)

As set forth with our newly appointed Joint Accreditation (JA) we now promote interprofessional continuing education (IPCE) activities specifically designed to improve interprofessional collaborate practice (IPCP) in healthcare delivery.

Mission for the Foundation for Care Management – a center for learning and change

We endeavor to directly and jointly provide programs that facilitate changes in patient outcomes.  We measure this through program evaluations and conduct outcome studies to gauge overall program success articulated through changes in knowledge, skills, performance and competence for individual learners and team based collaborations.

Evidenced-based outcomes expected of participants who complete FCM-provided programs include:

  • Enhanced team based learning as a result of current continuing education and community engagement in broad-based knowledge
  • Effective and accurate treatment of diagnosed conditions, e.g. accurate and appropriate pharmacotherapy leading to decrease in adverse responses by patients and improvement in long term health
  • Improved patient and/or community health or well being
  • Improvements in technique and skills strategy based on current literature and peer to peer support to enhance the work of the team and improve patient outcomes
  • Continuous quality improvement related to information gathering and analyzing of data for improving outcomes and follow up on behalf of FCM

To ensure expected outcomes FCM engages in continuous quality improvement by

  • reviewing program results and making needed adjustments to ensure its mission is being met to its fullest extent
  • conducting weekly internal meeting to review internal systems and workflows to increase efficiently as it relates to information gathering and analysis of information
  • ongoing education through the ACCME Academy and other educational offerings

Continuing Medical Education

For Physicians, Nurses, and other healthcare providers who are needing continuing medical education that is convenient in time and easily accessible. 

Accreditation

Working with organizations vested in a team approach with physicians, nurses, pharmacists, and other allied health professions to facilitate improvements in patient outcomes.

Evidence Based

To fulfill the mission of evidence-based course content, FCM peer reviews all potential lectures and programs to ensure that the content is science-based with published research and has content validity, balanced viewpoints, and free of commercial bias.

The Foundation for Care Management is a 501c3 designated as a nonprofit organization approved by the Internal Revenue Service as a tax-exempt charitable organization registered in the state of Washington.
