INSTRUCTIONS FOR CLAIMING CME/CE CREDITS
Step 1. Select the Claim CME/CE Credit button above.
Step 2. If Previously Registered with the FCM website, enter your account login information in the fields provided on the returning page and "Sign-in"
Step 2a. If Not Previously Registered with FCMcme.org, select “New User" and complete the website registration form and "Submit Registration".
Step 3. On the returning page enter the "Course Code" provided and “Submit".
Step 4. The program "Post Exam" will display, complete and “Submit". An immediate grade report will display.
Step 5. Upon receiving a passing score select “Continue" and complete the program Evaluation and “Submit".
Step 6. Click the green "Print/View Certificate" button displayed directly across from the program title.
Note: You may return at anytime in the future and access your previously claimed credits by entering the email address and password submitted in Step 2 above.

This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of the Foundation for Care Management (FCM) and What’s New in Medicine (WNIM).
The Foundation for Care Management (FCM) is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.
FCM designates this educational activity for a maximum of 18 AMA PRA Category 1 credits™. Physicians should only claim credit commensurate with the extent of the participation in the activity.
